request-queue-5: - Provide more scalable block level I/O (SMP or
multiple disks). Per queue request tables with
O(1) time for getting a new free request,
only wake appropriate devices when waiting
for a buffer / page or free request.

request-table-1 - Light weight version of above, uses two (three,
extra for loop/nbd) global request queues. Still
gives O(1) time for new request.

tables, elevator updates, etc. Ala request-queue
patch, but with io_request_lock still in place
and without the b_queue unplug changes.
