Fixes:
DAC960/cciss: implement own end_that_request_last, they need to
call blk_finished_io() themselves. Also remove
redundant same_segment.
cpqarray: same as above, wrong segment count fix too.
ll_rw_blk same_segment, and document blk_finished_io for
end_that_request_last
